We use Lucy Calkins Writing Workshop.  Here is the format I use for my
conferences:

Ask student:  What are you working on?/Where are you at?
Give a specific compliment.
Teaching Point:  Find something the student needs to work on and do a
quick little mini-lesson on the spot with them.
