I don't teach in a middle school, but I've taught 6th grade for the past two years. I will admit I don't do things all that differently in 6th than I did in 4th. Our conversations are just different becuase of their age. I still do mini-lessons with my kids gathered on the floor and me with my easel. I still do guided reading and individual conferences, although not quite as often as I did them in 3rd and 4th. I also do a lot more lit circles.
A great resource for your struggling readers is Kylene Beers' book, When Kids Can't Read, What Teachers Can Do. That's the only one I can think of off the top of my head!
Good luck, let me know if I can be more specific for you...like I said I'm in an elementary school, so I had a lot more freedom (with schedules and such).
